Just as a reminder of school dental and physical requirements:

• The Pennsylvania Department of Health requires that all students receive a dental exam upon original entry into school (Kindergarten or first grade), while in third grade, and while in seventh grade. An exam that has been done after July 1, (one year prior to the required school year) is acceptable. Please submit Private Dental Form to the health office, via community portal, or fax to 570-458-4715.

An exam that has been done after July 1, (one year prior to the required school year) is acceptable. Please submit Private Physical Form to the health office, via community portal, or fax to 570-458-4715.

For those students entering 7th grade for the 2022-2023 school year...

The Pennsylvania Department of Health requires students entering 7th grade receive the following immunizations before the first day of school:

TDAP (Tetanus, Diphtheria, acellular pertussis) and MCV (Meningococcal conjugate vaccine)

For those students entering 12th grade for the 2022-2023 school year...

The Pennsylvania Department of Health requires students entering 12th grade receive the following immunization before the first day of school:

MCV (Meningococcal conjugate vaccine)
